Feline Hepatik Lipidoz İçin Bir Tanısal Araç Olarak Shear Wave Elastografi: Bir Olgu Sunumu

Özet

Hepatik lipidoz (HL), ayda sık görülen bir hastalık hastalığıdır. Histopatoloji tanımak için altın standart olmaya devam etsin de, invaziv olmayan tanısal araçlara yönelik artan bir durum bulunmaktadır. Bu olgu sunumunda, HL tanısı alan geriatrik bir kedide hepatik parankimal değişiklikler Shear Wave Elastografi (SWE) ve Bilgisayarlı Tomografi (BT) olarak değerlendirilmiştir. SWE ölçümleri, sol lobda 30,42 kPa ve sağ lobda 19,68 kPa hassasiyet aralığı sonuçları ortaya koymuş olup, bu değerler sağlıklı yollar için başlangıç ​​referans aralıklarının belirgin şekilde üzerindedir. BT görüntülemede, lipid infiltrasyonu ile uyumlu olarak azaltılmış hepatik zayıflama ve hipodens kapasitesinin çalıştırılması. Önkontrast fazda beyin ve dalak zayıflama değerleri sırasıyla 44,37 HU ve 55,84 HU olarak ölçülmüş ve bu değerler, insan literatüründe steatoz iletilmiş olarak mevcut değerler altında olan 0,79'luk bir beyin/dalak (K/D) oranı ile sonuçlanmıştır. Kontrastlı portal venöz fazda da benzer şekilde 0,82'lik bir K/D oranı izlenmiştir. Histopatolojik incelemede SWE ve BT ile uyumlu olacak şekilde Evre 3 hepatik lipidozu doğrulanmıştır. Bu olgu sunumu, HL'li bir kedide SWE'nin ortak sunumu gösteren ilk rapor olup, klinik pratikte invaziv olmayan ve kantitatif bir araç olarak potansiyelini ortaya koymaktadır.
